Diagnosis: Sphenoid mucocele

The sphenoid sinus is the least likely place to give rise to a mucocele. The frontal and ethmoid sinuses are the sites of 90% of paranasal sinus mucoceles. Mucoceles of the sphenoid tend to expand into the apex of the orbit and the posterior ethmoids. On the more inferior images, this could be a pituitary macroadenoma . However, as you look more superiorly, the mass ends at the sella and has its epicenter in the sphenoid sinus. Malignancy is much less likely due to the fact that most of the surrounding bone is remodelled rather than destroyed. Related Cases
