Diagnosis: Ependymoma

Spinal cord ependymomas are the most common spinal neoplasm. They typically enhance heterogeneously and are well circumscribed. Hemorrhage and necrosis are common. The average age at presentation is in the 40s with LBP or radicular symptoms. Astrocytoma is much less likely here since they tend to be poorly defined, enhance less and usually occur within the thoracic or cervical cord. Cauda schwannomas can resemble small filum ependymomas, but would not achieve this size. Meningiomas are also in the differential for intradural masses. They are in older patients, are dural based, and tend to occur more in the thoracic and cervical spine. Related Cases
